Geralt spares Aryan, allowing his mother, Maria Lousida la Valette, to respond positively to Geralt.
Geralt escapes Flotsam with Vernon Roche, which allows the Temerian Partisan Camp to respond positively to Geralt.
Geralt helped Roche rescue Anais in Loc Muinne, which means Carduin of Lan Exeter never appears, but this has no consequence on your relationships with Vernon Roche or Triss Merigold.
Geralt left Sile de Tansarville to die, which means she doesn’t appear briefly in “The Great Escape” quest.
Geralt killed Letho, which means you won’t have access to the “Ghosts of the Past” side quest.

Should You Bow or Don’t Bow in Imperial Decree?

After answering the questions, you will meet Emhyr var Emreis, the emperor of Nilfgaard. You are given the choice to bow or not bow, and while you’re neutral in the war, it’s recommended to bow so you don’t get the chamberlain in trouble, as he’s just trying to do his job. Regardless, there’s no negative consequence for Gerlat or the story no matter the choice.
